Navigating new horizons: Launching services to support St Helena Hospice with compassionate end-of-life care

At Radfield Home Care Colchester, Frinton & Clacton, we take immense pride in our role within the community, supporting and collaborating with other care providers to deliver the highest quality of support. Over the past six months, we have had the honour of developing service offerings to support St Helena Hospice in Colchester.

The team have worked hard to develop services that provide valuable support for compassionate end-of-life care, care visits overnight, and a dedicated night-sitting services. These services are a testament to our collective dedication to providing comprehensive, person-centred care that caters to the unique needs of clients and their families.

Compassionate end-of-life care

Launching a compassionate end-of-life care service to support St Helena has been an immensely rewarding experience. This service is designed to provide personalised care for people in the comfort and familiarity of their own homes. We have developed the following services:

Developing personalised care plans

We have worked closely with St Helena Hospice’s healthcare professionals and families to help create individualised care plans that respect the wishes and needs of each person. These plans are designed to be dynamic, evolving with a person’s condition and preferences to ensure the most compassionate and respectful care.

Community engagement

Our efforts have included engaging with local communities and healthcare providers to raise awareness about this vital service. We have supported informational sessions and facilitated partnerships with other healthcare organisations, ensuring a seamless continuum of care for peoples requiring end-of-life support.

Care visits overnight

Recognising the critical need for consistent care during nighttime hours, we have implemented overnight care visits. Here are some key aspects of our support:

Staffing and scheduling

We have expanded our team to include Care Professionals who specialise in overnight care. By creating flexible schedules, we ensure that our staff are well-rested and ready to provide attentive care throughout the night. This entails planning, training, and managing schedules to maintain the highest standard of care.

Person-centred approach

Understanding that nighttime needs can vary greatly, we have developed and delivered a flexible approach to care visits. Whether a person requires assistance with medication, mobility, or simply a comforting presence, our Care Professionals are prepared to meet these needs. Our involvement helps ensure that each person’s specific requirements are met with empathy and professionalism.

Technology integration

To enhance communication and coordination, we support the use of a digital system for tracking care activities and client conditions in real-time. This system allows Care Professionals to provide detailed reports, ensuring that family members are kept informed and that the highest level of care is maintained throughout the night.

Night sitting service: providing peace of mind

Our collaborative efforts have also extended to the night sitting service, which offers a continuous presence through the night, providing clients and families with peace of mind. Our support has included:

Recruitment and training

We have recruited compassionate Care Professionals with the patience and dedication required for night sitting. Training programs focus on overnight care techniques, emergency protocols, and maintaining a calm and supportive environment, ensuring that all Care Professionals are equipped to handle the unique challenges of nighttime care.

Customised care

Each night-sitting assignment begins with a comprehensive assessment to understand the client’s specific needs. We aid in creating personalised care plans, ensuring that every client receives the support they need. This personalised approach is essential to delivering the highest quality of care and addressing the unique needs of each client

Family support

We recognise that nighttime care can be emotionally taxing for families. To alleviate this burden, we provide additional support and resources, including regular check-ins and a 24/7 helpline for immediate concerns. Our involvement helps ensure that families feel supported and confident in the care being provided to their loved ones.
